The Needle That Never Deserved the Blame
As she lay on the operating table, her hands trembled more from fear than from pain. Im not afraid of the surgery, doctor, she whispered. Im afraid of the injection in my back. My neighbour says she has suffered from back pain ever since her caesarean section. It is a conversation anaesthesiologists hear almost every day. A few minutes later, spinal anaesthesia was administered. Soon, the operating room echoed with the unmistakable cry of a newborn. Tears rolled down her cheeks as she heard the sound every parent longs for. The surgery was uneventful, her recovery smooth, and within days she returned home with her baby in her arms. Months later, she began experiencing occasional backache. She attributed it to the spinal injection. Yet her days were now filled with lifting an ever-growing infant, bending over the crib, carrying the baby for hours, breastfeeding in awkward postures, and coping with sleep deprivation. Like many mothers, every ache reminded her of - not pregnancy, not childcare, not altered biomechanics but the tiny needle that had been inserted into her back. The spinal anaesthesia had become the silent culprit. But was it really? Medicine often battles myths that persist despite overwhelming scientific evidence. Among the most common is the belief that spinal anaesthesia for a caesarean section causes chronic or lifelong back pain. The evidence tells a different story. Pregnancy places extraordinary mechanical and physiological demands on the spine. As the uterus enlarges, the bodys centre of gravity shifts forward, increasing lumbar lordosis and altering spinal loading. Pregnancy hormones such as relaxin and progesterone increase ligamentous laxity, reducing joint stability while preparing the pelvis for childbirth. In addition, the abdominal wall stretches and core muscle strength diminishes, reducing spinal support. These changes, coupled with postpartum childcare, prolonged feeding positions, repetitive lifting, and sleep deprivation, make low back pain extremely common during pregnancy and in the months after delivery. Studies estimate that 5070% of women experience low back or pelvic girdle pain during pregnancy, and a substantial proportion continue to have symptoms for several months postpartum, irrespective of the mode of delivery. Large prospective studies and systematic reviews have consistently demonstrated that spinal anaesthesia is not associated with an increased risk of chronic back pain when compared with women who receive general anaesthesia or no neuraxial anaesthesia. While a small number of patients may experience mild tenderness or soreness at the injection site for a day or two due to needle insertion through the skin and ligaments, there is no convincing evidence that an uncomplicated spinal anaesthetic causes persistent structural damage leading to chronic back pain. So why does the myth endure? Part of the answer lies in human psychology. When two events occur close together, we naturally assume one caused the other. Since the injection is administered in the back, any subsequent episode of back pain is often attributed to it, even though the underlying cause is more likely to be the normal musculoskeletal consequences of pregnancy and the physical demands of caring for a newborn. Ironically, this misconception sometimes discourages women from choosing one of the safest and most effective anaesthetic techniques available for caesarean delivery. For most women, spinal anaesthesia is the preferred technique because it allows the mother to remain awake during the birth of her child, provides rapid and reliable surgical anaesthesia with excellent postoperative pain control, reduces airway-related risks associated with general anaesthesia, and facilitates immediate skin-to-skin contact and early bonding with the newborn. Every successful spinal anaesthesia tells a story of safety. It is the story of a mother hearing her babys first cry instead of waking up after the birth. It is the story of modern anaesthesia making childbirth safer for both mother and baby. Perhaps it is time to rewrite another story as well. The next time someone says, My spinal anaesthesia gave me lifelong back pain, we should respond with empathy, careful evaluation, and scientific evidence. Back pain deserves to be taken seriously but it also deserves an accurate diagnosis. The real story was never about the needle. It was always about a mothers remarkable journey and our responsibility, as doctors to ensure that fear is replaced with facts.
